I hereby give permission for my child ______________________________ to participate in the Bullwinkles End-of-Year Celebration .  I give permission for my child to be transported in the personal vehicles of Good Shepherd Youth Ministries staff or volunteers.  I agree to hold harmless Good Shepherd Youth Ministries, its officers, and/or employees for any accident or injury resulting from my child’s participation.
